What to Do
The beginning of the counseling process involves filling out several different forms of paperwork that are required by law and also help the counselor in completing a mental health assessment. By filling out paperwork prior to your first visit, it will ensure the limited amount of time will be optimized in getting to know you rather than on filling out paperwork.
To obtain and fill out this information, when scheduling your first visit:
You can have your documentation emailed to you by your counselor, in which you’d print the documents, fill them out, and bring them to your first visit.
You can arrive a half-hour prior to your first visit and fill out the documentation, given to you by a counselor or staff member
You can request access to our New Clients tab on our Navigation bar, in which you’d download, print, fill out, and bring the documentation to your first visit.
Our documentation will ask you to share your reasons for counseling, to give a general personal and family history, share any physical symptoms, help you identify your strengths, identify ways you have coped through difficult times up to this point in your life, and goals you hope to reach at the end of the counseling process.
Don’t have any idea on what your goals or what you want to work on in the counseling process? Don’t worry! Your counselor will help you during the first few sessions to make a plan and how you can both collaborate together to reach the goals you’ve identified together!
